This short questionnaire helps our attorneys prepare for your initial consultation. Your answers are confidential and will be sent securely to mjs@Sweeney-Law.com. Take your time — you can complete it in one session. Most clients finish in about 10–15 minutes.

Please identify the people you want to manage your estate and affairs. You can name the same person for multiple roles. For each role, provide a name, town, and state.

Executor / Executrix Manages your estate after death
Health Care Representative Authorized to instruct physicians regarding life support
Power of Attorney Manages your financial affairs if you become incapacitated
Conservator of the Person Day-to-day care if you become incapacitated
Conservator of the Estate Financial management if you become incapacitated

If you are on life support in a permanent vegetative state, do you want artificial life support discontinued?

Real Property

Tell us about the real estate you own. Approximate answers are fine — we can gather precise details later.

Section 3

Liquid Assets

Financial accounts, insurance policies, and investment portfolios. Approximate answers are welcome.

Personal Property & Debts

Vehicles, valuables, and any outstanding personal obligations.

Section 5

Additional Matters

A few more questions to help us fully understand your situation and planning goals.

This threshold determines Connecticut estate tax exposure.

Section 6

Will & Distribution Planning

Please describe how you would like your property distributed at death, including any specific bequests and your wishes if an intended beneficiary predeceases you.

Children & Guardians

If you have minor children, this section designates who will care for them and manage their assets if both parents are unable to do so.

First Responder / Temporary Guardian Cares for children immediately if both parents are incapacitated
Permanent Guardian of the Person Raises your children if both parents die
Guardian of the Estate Manages money & assets left to your children
